
PTO REQUEST FORM
PTO POLICY (Outlined in Employee Handbook)
Eligible Full-Time Employees earn: Twelve (12) PTO days (total: ninety-six (96) PTO Hours) from January 1-December 31 + (1) Birthday PTO Day/eight (8) PTO hours (to be used any day during Employee's birth month)
PTO can be requested in a minimum of (30) minute increments
Partial Day PTO REQUIRES the Employee to clock in/out upon departure/return to and from work.
PTO requires a two (2) week advance form request for any non-emergency or illness
All PTO needs to be exhausted prior to any unpaid time off is taken into considered for approval
PTO requests lasting longer than one (1) consecutive day during the high seasonal months of October - May, require a twenty-one (21) day advanced form request.
PTO days will expire at the end of each calendar year. No payouts or carry over of PTO time.
If the Employee has an unexpected need (i.e. sudden illness or emergency) for PTO, he/she should notify Nicole Geraghty as soon as possible and contact each day of absence. A PTO Form is required to be submitted upon return to work.
HOLIDAY POLICY: PTO days requested surrounding Paid Holidays (Outlined in Employee Handbook)
Only one (1) of the pre-holiday or post-holiday day(s) can be taken for the Paid Holiday to count towards the Employee’s paycheck for the respective pay period.
It is highly discouraged, but a consideration will be taken if a valid reason arises and the employee needs to take both pre-holiday and post-holiday days surrounding the Paid Holiday, the Holiday day(s) will go unpaid.
